Anybody have experience filling out Aflac short term disability paperwork?

Anybody have experience filling out Aflac short term disability paperwork?
I feel like it's so darn complicated!
Do I basically have to send one form to my doctor, and another to my job, have them fill it out, send back to me and then back to AFLAC? Such a PITA.
Would anyone mind walking me through this?  TIA

yes, you need to have your job just fill out the employer area...and your dr fill out the medical necessity part, and then fax it in to the number at the bottom of the page..it should take about 10 days or so to get your first check

i filled out my part- gave it to drs office- then gave it to employer - they did their part- and they were nice enough to mail it in for me...
