Name Change cheat sheet?
I am currently changing my name back to my maiden name with credit cards, frequent flyer numbers, etc. I have made a standard package that includes my my driver license, divorce decree, and standard letter but it seems each credit card, etc. has different protocol. Has anyone seen a phone number/fax number cheat sheet for this? If not, I'm making one after my 2nd experience changing my name in 4 years!

If she changed her name frequently she would know how to do it already and not require a "cheat sheet". How about a little compassion, or next time remember what your foot tastes like so you don't put it there again.

I believe once you change your name with the DMV and Social Security that should be all you need in order to change everything else. I haven't worked at the DMV in 5 years, but I would assume certain ID standards haven't changed. You should change your name with SS first then DMV.

And FWIW, I am sure the OP meant from going from her maiden name to married name and then back again to her maiden name (twice).
NOT that she's moving on to a new person every year........

I don't even remember this process and what was needed when I changed to my married name.
Lots of companies are different in what they require.

Haha, wow I started a fire storm! Didn't expect that! Thanks to the ladies on here that were able to clarify what I meant in my apparent cryptic request. 
Yes, I am going back to my maiden name. What I meant by a "cheat sheet" was a list of phone numbers/fax numbers/emails to contact when changing legal last name. Not only credit cards (those were the easy ones), but I travel often so I have frequent flier numbers, hotel reward plans, etc. that I needed to update with my new name. Unfortunately most of the websites are not clear and you need to sit on the phone with computer operator on a 1-800 number just to get the fax number or email address to send paperwork. So anyways, I was just looking for a info sheet like this!
Thanks for the help--I created one using information I found on internet as well as my own research so that it can be easy for future ladies (whether due to divorce or marriage) to do!
I counted and in total, I just changed 16 airline/hotel/car rental frequent flier programs so yeah, regardless of how often I may change my name, it is a b*tch to do regardless.

I have one of those "sheets" just in case I lose any of my information or God forbid my wallet.
I keep a copy in my desk at work.
Nuisance to change one's name, but you did yours for a good reason.

It truly is! I am shocked that most of my airline programs need notarized letters on top of all the support required at DMV, SS! Good luck to you too!
