Casa Rustica in Smithtown did a nice job. Don't know the prices. Maybe try Mama Lombardis also.

We had DS' christening party at The Oar in Patchogue. It's an entirely separate building from the main restaurant so you have privacy. We paid $25pp. We were very happy with the room, the food and the service.
