I am the annoying person that refuses to use nicknames  I hate any and all variations of my name (ironic since it is my username) so I always use everyone's FULL name.
Mike is Michael
Josh is Joshua
Chris is Christopher
At my old job I worked with a lot of younger guys and they always said I reminded them of their mom because of that, LOL
If I am close to the person I will make their name Italian and call them by that!
I am the only person who calls my DH Anthony really. He is AJ or Ant to his family and Tony to his friends
You mean if someone specifically asked you to call them Mike, you'd call them Michael?
If someone told me personally "Hey, call me Mike" of course that is what I call them ( my BIL is Mike in fact ) but if they are just introduced casually as Mike, I will call them Michael unless they say something about it. Just a strange thing I have. A few people told me that they think its sweet LOL

Ugh - my Father does this and I keep telling him it's so rude !! Liz is LIZ, NOT Elizabeth, Kathy is NOT Kathleen, Dee is NOT Diana ........he has done this to my friends (Only the girls though) since I was a kid !! -

The first time I ever met any of my neighbor's in NC they started to call me "Em" and "Emmy". It felt like kindergarten all over again! Do you have any idea how long I was an Emmy? Day one of kindergarten to day one of college! 
There is not a timeline to give a nickname just make sure the person receiving it does not mind.

I only do that to my brother!  I figure that's allowed. I try & call people by what introduce them selves.
Sometimes, though, if a 3rd party is inroducing us, they will use the nickname and I use that not knwoing that the other person hates it.
I was introduced to someone as Jen,--everyone called her Jen, I lived with her for 3 years, always called her Jen, and then I fiond out that she hates Jen, prefers Jennifer, and pointed out that she never introduces her self as Jen.
I feel bad, but I've known her 13 years--it is too late to switch now!
Same thing happened with my boss, but she admits that it's too late to get people to call her by her full name. I try to introduce her to new people by her full name.
I HATE when people shorten my name. It happens at work all the time,and my name doesn't even really have a nickname.

LOL oh no, I try to remember what people like to be called because I am so sensitive about it!
My nicknames either sound like a man's name ( Mat ) or a little girl ( Tilde ) so I hate when people use them. Tilly reminds me of the Beastie Boys. . . I had ( and have! )issues.
The only person who gets away with a nickname is my mom - she calls me Matildina sometimes.

When I first met my husband, he introduced himself as Sebastian, but then said "Seb". Most friends call him that but his mother always says Sebastian. I find it very odd to talk to her about him because I'm not sure what to call him!
My name is pretty difficult to shorten, but somehow I've ended up as "Fazz" or "Fazzah"here in the UK I would be super weirded out if someone I didn't know VERY well started calling me that.

I never thought about an actual amount of time but I do agree it's nice for a person to ask. I always introduce my self as "samantha" and most people automatically call me "sam". I don't mind it, I actually never really heard people use my full name until college. (unless i was in trouble of course...then the middle name came out too lol)
"sami" is reserved for close friends & family so it annoys me when people I hardly know use that.

I always ask them, "Does anyone ever call you ______?" and that usually leads to the conversation on what nicknames they like and don't like.
A guy at work, his name is Mohammad. The second day he was working there, I asked him if anyone ever called him Mo. He laughed, a lot, and said no.
Meanwhile, 5 months later, I've heard everyone call him that at one point or another.  I think though, with him, he feels more accepted with his nickname.
I don't go by my full name, ever. There are too many variations for the nickname and people usually get it wrong and call me Pat, Patty. I do respond to Trish and Tricia but call myself Tricia.
Then lovely ladies who only call me Greenie, even in person. I don't mind it at all, I like having a nickname. I call them by their screennames too.
