You have to go to civill court and petition for a name change. I know about how it works in NYC but not too familar with the process in your particular county.

No you have to go to the one in Manhattan by Chambers Street. Let me get you the link for the information.
In a nutshell you fill out the paperwork and get it notarized. You bring it to the filing clerk and pay whatver fee (money order only), then they give you a date that you come back and they tell you if it was approved or not (the hearing).
I am going to try to find the link now.
