question about fmla/disability paperwork

i was just given my paperwork yesterday for the fmla,disability and the leave from my job.
my EDD is 9/26 and i want to work as long as i can. I put down that my last day would be 9/21 (the friday before) but if for some reason i go later than that and i feel okay then i think i want to keep working!
My dr said she'd let me go 2 weeks past my EDD. but if it is 2 weeks later, then thats 3 weeks i'm sitting at home doing nothing when i could use those 3 weeks with the baby,since i'll only be out for 12 weeks.

Do you know if i can change the date on these forms?

My job lets you change it. My doctor initially put my due date on the forms and now gave me a note overriding it.
