going away on vacation and need help with wording for letter of care for children

my mom wants me to notorize something saying i give her permission to take care of the kids in my absence. is there an official letter that needs to be notorized? anyone have the wording?

there is nothing "official" that has to be used, but here is what I have used in the past:


MEDICAL CONSENT FORM



We, INSERT NAMES, parents and natural guardians of INSERT NAME, hereby authorize INSERT NAME and/or INSERT NAME to give consent for all medical and/or surgical treatment that may be required for our child during our absence from INSERT DATE up to and including INSERT DATE.
