Getting Your Records from RSofNy

I was wondering for those who left that practice, how soon you got your records ?

I called, and they said I have to write a release paper and my dh has to sign it as well. I am unsure what to even include in this release paper ???

The receptionist said it takes 2 weeks ??

I have an appt with a new RE jan4th...so I guess I should do this paper release ...by tomorrow ? I am confused by that place as always ...

I would appreciate any information on this, thank you very much.

Basically all you do is write a letter requesting your records be sent to you home. You sign and Dh signs then fax it over. I would also send the hard copy. Sometimes, you can get your records faster if you are willing to go pick them up.

Don't worry about the new doctor appointment because if you don't have your records by then, you and DH will sign a HIPPA form and they can harrass RSofNY until your records come.

I was at RSofNY this Saturday and had the thought that possibly many women are doing this as they have a sign up concerning additional copies of records (Second, third etc. copy) will be charged 75 cents a page.

I replied via FM, but I asked for my records. I faxed a medical request for the records to be sent to my home address.

Redtstar- hey told me two weeks too and I got them within the week

I haven't been there since Sept, so I have no up to date info - just wondering

I simply called, and they faxed them over that day. Maybe because I had already been "released" from their care and wasn't going to another RE?

I FM'd you but so everyone else knows.

I faxed a ltter stating what I wanted.
DH and I had to sign a consent in person (not faxed)
They made the copies then and there.
It was maybe a week after my request but they didnt do it until I was there so it took all of 5 minutes.

If they have a sign up then it does sound like a lot of people are leaving. They need to do some major reorganization!

Do they ask you what you are needing them for?
I do not want them yet to know Im leaving but I want my records now.

I didn't go there, but when I requested my records from my old OB/GYN, I just said I wanted to keep them in my own personal records.

They didn't ask me a single question, in fact they tried to rush me off the phone. I was told abruptly what to do.

Please release a copy of all my medical records, including but not limited to, progress notes, blood and laboratory results and diagnostic tests.

Please send medical records no later than xxx The medical records should be sent to me at the above address.

By my signature I authorize release of Medical Records.

I don't know, when I was discharged, they had me sign off that it was OK to give me copies of all my records. I went there, signed the release and they had the copies ready for me.
