Does anyone know the process for getting someone into a group home?

She's 20 and an addict - will do anything she can get. She has a bad home life and a major anxiety problem. Drug addict father, abusive mother.

Her mother dumps her in and out of different hospitals just to get rid of her. She doesn't have any kind of education. Her mother "homeschooled" her, but never taught anything. Mom discontinued lessons when kid was 14.

She has no friends she can safely move in with - they're all friends from the worst parts of her life.

She's south shore suffolk, but will go anywhere she can get in. She actually just told me that she's considering packing a bag and living @ a train station just to get out of that house. She has no money, no job, no skills, and her mother holds her meds - she can't leave without them.

Anyone know the process?

I don't know the process for an adult, but I wanted to give you this link to Phoenix House. They have residential drug treatment/mental health programs. Maybe somewhere there would be able to guide you in the right direction. Phoenix House

Look into Seafield (http://www.seafieldcenter.com/Seafield/index.cfm). They have in-patient and out-patient programs.

she's tried that- she's looking for something on more of a long term basis. Her parents' home is her #1 toxin.

If she's thinking about suicide, she need's hospitalization sooner than later. The discharge options after that can be determined after she gets healthy.

I would say Seafield as well..

but if your looking for something more long term.. there is also Teen Challenge (I think it's in the city).. they stay there for a year or more

I don't think she has insurance available to her, either.

I am NOT a social worker, but I believe she'd have MUCH better chances of getting into a group home after completing a residential treatment program... Not even sure a group home would take her otherwise (but I'm not sure).

It does sound like she needs a residential treatment program first anyway...

yes..there is a process. Doctors need to be involved, social workers, psychiatrists..etc..it needs to be a team approach not just relocating someone.

Does she have a doctor?

If she has a psychiatrist prescribing meds for her, this would be the best person to start the ball rolling on her getting other services. It's difficult, b/c she's not a child, so you can't go through CPS, and she doesn't have children, so she can't get preventive services (at least that I know of, but I don't know Suffolk's system). There is a number for reporting adult abuse in NYS. Typically, it is elder abuse that is reported, but this is worth a shot, as she has been a victim of abuse/neglect since childhood and is trapped in this situation.

Protective Services for Adults

The other thing that I can think of is for her to try to get an intensive caseworker through the Office of Mental Health. The application is lengthy, and requires a treating doctor to complete portions of it. Here is a link to some information about agencies that provide caseworkers in suffolk county. NYS OMH

The way the system works is that you have to have some kind of open case and caseworker in order to get services. A group home may be the end result, but there will be many steps between points A and B, and unfortunately, it's not always easy.

or just call some centers directly & just ask how you would have to go about it.

I'm surprised there was no discharge planning when she was released. It's tough to get a group home placement, but I would think that there would be a social worker there to help her find some kind of supportive housing, or to at least connect her with the appropriate outpatient services.

She should have been given discharge paperwork when she left the hospital. Maybe that would give you someone to contact?

Teen Challenge does take people in their early 20's.. not sure about the insurance aspect but they do have a really good success rate

She definitely needs to be admitted inpatient at the hospital especially because she is expressing suicidal thoughts. Once she is there the hospital has the obligation to get her services especially if she is considered "homeless" therefore her mother needs to NOT accept her back home. As tough as that sounds the hospital cannot release her un-domicile. If they see that she has somewhere to go they will release her once they feel she is stable and more often then not it's too soon.

Once she gets admitted the SW at the hospital will have to complete applications for an ICM (intensive case manager)and can complete paperwork to the Long Island Single Point of Accesss (SPA) office. This application goes out to all agencies on the Island who have openings in their group homes.

The hospital should also apply for Medicaid for her because most of the group homes accept this as payment source and receive their funding through medicaid therefore she has to have Medicaid. They can accept her on Public Assistance while applying for Medicaid.

It's a pretty long process but the important thing is to get her hospitalized and meet with he hospital for discharge planning meetings.

I previously supervised and Apartment program for adults 18 and older for many years and a very good friend of mine is an intake manager for this residential organization and closely works with the single point of housing office. I wish i saw this post sooner.

I agree with the above poster, although I know a lot of hospitals will discharge regardless of having a place to go.


I also know that housing can be scarce out there, but it is definitely worth a shot.
