Doctor's New Plan

My doctor called me after the surgery to talk about how my right tube was kinked and twisted with scar tissue, and he was able to fix it.

Now he wants us to try on our own for 3-4 months.

This has me reeling a bit. After 2 failed IVF cycles and 3 CP, I find it hard to believe that my tube was the problem all along. The thought of going back into the dark of OPKs that only work inconsistently and the drudgery of timed intercourse makes me want to cry. I did cry.

His logic seems to be that the crazy hormone environment of "all these gonadotropins" is not producing the greatest embryos. I have proven to be a poor responder on injectable medication, so I end up needing to take really high doses to produce multiple follies. But the first 2 CP happened when we were trying naturally. The 3rd was on clomid. My last doctor dismissed them as "bad luck." IDK.

This feels like a huge step backward into more wasted time. Yet turning to the big guns of IVF only burned through $15k of limited IVF coverage without so much as a CP or a frostie to show for it.

I feel so conflicted. I have a post-op with him on 11/18, and I feel like I need to get my head screwed on straight before I go in to see him so I know how to advocate for myself.

Thoughts?

I'm most certainly not an expert. But I would not accept trying on my own after going through all that youve gone through, especially chemical pregnancies. Not sure who your doctor is, but you may have read on here that chemicals could signify immune issues, which many docs don't believe in nor will they treat.

Also, I dont quite understand his reasoning. While your body is amped up on all the fertility drugs now, (even though i dont know your history) you obviously have been going for IF treatments for a reason. Maybe you were trying naturally for a while and it didnt work, or you have a specific medical reason for IF. I dont know, I'm not a doctor but it makes no sense.

You should definitely advocate for yourself at your next appointment. If your doctor is unwilling to continue trying with treatments in the next few months, I would suggest looking for someone else who is more in line with your thinking. JMO.

i think it depends what your dr means by "trying on your own". if he/she means that you should go with no help at all, yeah i would def argue that or find another RE.

however if he thinks that you should go with no gonadatropins since you dont respond well to them, but they are still willing to treat any issues that are causing CPs, provide progesterone post-O support, or even do some natural cycle IUIs, then i would probably opt for that option. why waste money on injectables if you are a poor responder and theres a chance you can get and stay pregnant without it?

We know every case is different
With that being said, I just had surgery also and both tubes were blocked. My surgeon wanted us to be super aggresive before possible IVF and start IUI after my 1 month healing wait.

It sounds like you might benefit from a second opinion which sucks in timing but it might put you at ease.

When I talked to him I reminded him that with the IUIs early on we learned that I don't necessarily O on time...like I'll have CM around day fourteen and wacky LH levels and end up with a giant overmature follie if I don't trigger. He said we could compromise--use the trigger and even do an IUI if I wanted. We have to have a conversation about progesterone because I also have a shaky LP (12 days, spotting, etc.) He was supplementing IUIs already, so I'd have to push that we continue to do so given the history of CPs.

Money/coverage is not an issue at all. I'm really lucky that way, and I'm sorry for all the girls who have to stress over finances in addition to this IF beast. I just have that lifetime max for IVF coverage, so it's important we don't burn through it without exploring the reasons why it's not working. I get that.

The thing with the injectables is that I have to take a lot of them to have a reaction. Weird because I was making as many follies on clomid as I did on follistim/menopur over the summer for that IVF cycle turned IUI because of poor response. I had a much better response for the September IVF cycle, but I was being pumped with so much medication, I think he thinks I'm making bad embryos because of that and that's why IVF's not working...even tho those embies LOOKED perfect.

I am in the exact situation as you. I have had multiple failed IVF cycles and have been told by 3 different doctors that I should try on my own for awhile. They said that some women cannot get pregnant with a stimulated uterine environment and have seen so many pregnancies when they were on breaks. I don't know what to believe anymore either. They keep saying that since I was pregnant before, but miscarried at 8 weeks, there is a chance.
If I were you, I would go back to IUIs since you said your cycle can be alittle wacky. I would do a simple clomid cycle. This way you will be monitored and it is not so aggressive on your body since you have been through so much. Chat Icon Chat Icon Chat Icon

I sent the nurse an email about this a month or so ago after he mentioned that one of the things they look for via surgery is inflammation because it can "introduce mediators that interfere with conception" (doctor-speak for bacteria and immune response that damage cells and embryos).

She just sent me an email back that I should consult with Sami David in the city. I'm reading his book now. But he doesn't take insurance. I have to remember to ask again what his thoughts are on this when I see him on the 18th.

Im pretty sure that dr tortoriello at sirm treats immune issues and he takes ins.

My doctor's not a huge fan of clomid because of what it does to the lining. We tried letrozole, which works the same way without the side effects, but I didn't respond.
