Technically, its proper business etiquette to do so. And since you don't want to burn bridges, I would recommend it - thanking them for the opportunity to work there, etc.

yes.
after you break the news in person you should send a letter to make it official
if they know to expect the letter then its alot easier

Ditto. I looked on About.com for some sample resignation letters. They were a good starting point to help me write mine.

Yeah, I did a verbal resignation and followed up with a formal email and copied HR.

I gave a verbal resignation and then wrote a very short but sweet email resignation for the files.
I basically thanked everyone for all of the opportunities they gave me while working there and told them how much I liked the job but that I was leaving to attend law school.
Generally, I like to leave places on good terms because you never know where you will meet people in the future or if they could help you in your career further down the road.
