no. just mail your passport and I dont remember if it is your original marriage certificate or a copy. but mail those 2 things and they will mail you back your passport. it will be stamped in the back indicating a name change.

Yes. It will be stamped that indicating what your old name was and what your new name is and the date.
When it comes time to renew your passport then you will get a new passport with your new name.
They say 6 weeks but when I did mine I think it took about a month.

Oh shoot good thing this post is here to remind me. I keep forgetting to get mine changed and I need it in about 6 weeks!

It's actually really disappointing...you get your old passport back with a note in the front that says see amendment on page whatever, you turn to the back and they've typed 'So and So has changed name to Blankety Blank on 00/00/00'... And I constantly have to remind check in at the airport that there's a name change!
