The Four Seasons in Maui. 100% hands down. I have heard the Four Seasons in Costa Rica is also incredible.
The Ritz Carlton in Paris.

For me - probably Cap Juluca in Anguilla.

Either the Wynn in LV
Borgota in AC
or the Seminold Hard Rock in Hollywood, FL.
I would say the Radisson in Aruba too, but its far from anything REALLY fancy. Just set on a beach in absolute paradise so thats definitely a honorable mention!

The Four Seasons in Bora Bora. We were originally supposed to stay at St Regis but it was closed due to being hit by a cyclone shortly before our trip.
Funny side note, Leann Rhymes was staying there with her boyfriend Eddie whatever at the same time as us.

Florence, Italy- Grand Hotel Villa Medici
Miami- Fontainebleau
I have stayed at other really nice hotels but those are the first that pop into my head

Plaza Hotel Lucchesi - Florence
Ritz Carlton - Miami

The Princeville in Kauai, HI. I think it's now a St Regis. It was wonderful.
These are really nice as well - I've stayed at these for conferences:
St Regis - Dana Pt, CA
Terranea in Rancho Palos Verdes, CA
La Costa Resort and Spa, Carlsbad, CA

Most swimmable beach and beach service without a doubt! Those loungers are almost as comfy as my bed!

The Excellence Playa Mujeres in Mexico.
I have heard all the excellence hotels were AMAZING though :)
ETA: the El San Juan in Puerto Rico. :) Loved that place too!
