Mine is from Legal and General- I went to our insurance broker (who happens to be my brother in law) and they shopped me around for the best rate for me and my needs
I didn't have to make a dr appt- they sent someone to our house to take blood pressure, height, weight and blood and urine samples

I have Liberty Mutual, DH has Prudential. No doctor appts but for DH a tech came to the house and did some bloodwork and weight, BP, EKG.

What life insurance company do you use
You only need to have a exam done with labs for pretty much any company. An examiner will come to your home or business, ask you some medical questions, take your blood, urine, height, weight and blood pressure and depending on age/amount an EKG.
If you are above a certain age and going for a high face amount, the carrier may require the exam be done by a doctor. Otherwise depending on age/amount requirements and what's on your paramed, the underwriter may request and APS (medical records) from your doctor(s).

We use AXA Equitable, was through a broker that is a friend of my Brother in Laws, rates were the lowest I priced out (this is for term life insurance). They sent someone to our house to do the medical exam.
