I just went to melville Kids dentistry on route 110. I search on healthplex site and found them listed.

i called them to get my member number. also that broadway smile in Massepequa I heard very good things about and they take 2 types of Healthplex but not mine. You can call them and give name and SS# and they will tell you if they take yours
