Mine charges $10.
But I've only had to had them fill them out durng a well visit, which with my insurance has no co-pay.

Wow, love my doctor office even more now - they don't charge and we have lots of forms.
If I had to pay, though, I think I would rather pay a yearly fee, rather than have to worry about it every time I have to fax a request in, the school nurse calls them, etc.

Ours does not charge either. Meaghan we use Grello too...I was there today & no charge (for her K forms & paperwork on my son).

My son's ped in queens used to charge $10 to fill out a camp form... They offered a generic one from their office for free.

We pay a fee like that but it means that we can have unlimited forms faxed wherever/whenever for the whole year.

wow, never heard of that, my ped doesn't charge for it and I have asked them for the form at least 3 times, they usually fax it to me if I can't get there to pick it up. I have asked them to fill out a form my DD's school gave me AND also have asked them to fill out and give me their forms , from the office. They never charged anything.

We pay $10 a year. I've needed several forms a year.
They will mail, fax or let me pick it up.
I don't mind paying it.

i dont understand WHY -- i understand it takes time however aren't you a patient there and that is part of their patient care if you need some stuff filled out to show vaccinations/care etc that the ped provided?
i really hope my ped doesn't start doing this - i would be really annoyed... the amount of times i bring my kids in for sick visits and them taking a copay along with what the insurance gives them should cover the cost of filling out a form

I think it's because insurance companies pay so little to doctors now that they need to do everything they can to make money. I recently checked some claims on my insurance company's websight and services that were billed at $140 were paid $18.49 by the insurance company plus my $20 copay.

I feel that filling out at NYS required form for school is similar to a receipt for the viisit. Proof that your child had the required vac's and is healthy to attend school. I do not feel a parent is responsible to pay extra for that. We pay insurance out of our paychecks and deductibles and co pays. The Dr. negotiates a fee with ins and thats what they get paid and I feel filling out a form or giving a "receipt" should be part of the visit. I did get my son's form filled out with out a problem and a fee...it took a mere 3 mins.

They usually charge for the forms.
Everybody goes where they feel comfortable. I don't like paying $40 for the year, unlimited forms but that's what it is and I really like the practice.

they wanted to charge me $15 to fill lout my sons 4th grade school medical form: hgt, weight , blood pressure, BMI Dr sig & stamp..thats it. Sorry thats fraud!!
