Form fee @ Pediatrician's office!
$15 to fill out school medical form.. thats
what my Ped office tried to get me to pay
today at my kids (2) well visits...
paid $45 in co-pays and
on top of that they wanted $15 for each
child a year to fill out forms....I was shocked
and told them Im not paying that. anyone
else's get that at Dr office? is this new? Im
there once a yr and found it a lil ridiculous
..takes 2 min so fill out !! vent over

I switched practices over this. They charged me for a form and would not fill it out at the visit while I waited, nor would they fax it when I asked them to, and they wouldn't mail it either. They wanted me to schlep all the way back over there to get it. That was the last straw.
ETA, our new practice does not charge for forms.

Sorry to say it seems that they are all charging something these days. Our old office had charged $5 but then it went up to $10 or 15. Our new one charges I think $20 (and they didn't even fill out the WHOLE form - they left the part where it asks about allergies etc empty and only filled in the shot portion).
When I was looking for a new office last year I found that most charged anywhere from $10-25 a form. But there was one that I think charged something like $80 a YEAR for unlimited forms.

Wow, my office doesn't charge anything for forms, at least not as of this past September. I've never heard of that!
FWIW, we go to Grello Pediatrics in West Islip.

I went to 2 practices and both charged a fee.
The current one is $40 per year for 1 child. But that also includes Rx call in to the pharmacy.
I have a lot of forms to fill out every year.
It's VERY common for PED offices to do that.
If you look at the amount of kids they serve with school forms, camp forms, EI (OT/PT/Speech) forms.... that's a lot of forms for a PED to fill out.

My Dr charged me $25 to fill one out. I ended up leaving there because they charged me for a visit when they requested me to come in as there was a mix up with my meds. I was sooooo freaking mad I was shaking on the phone with the office mgr. My DH who has known me for 12 + years was like I have never seen you so mad.

I pay $80 total a year for both my kids for forms.....ridiculous!! If we didn't do the "program" it would be $20 per form.

My pediatrician's office charges a certain amount for the whole year. I think it's $15.
The supervisor from Early Intervention told me that doctors are not allowed to charge to fill out EI forms. I told my doctor's office this and they said they had never heard of it and they insisted that I pay.

mine charges and did NOT charge for EI forms, so this may be true.

Ours doesn't charge for forms and if they did it would have been reason enough for me to start looking for a new doctor. I think that's just unreasonable greed, plain and simple.

Wow. Thats shocking! I have been to see my GP about 5/6 times over the last month with myself and the kids and have had amazing care and haven't had to pay a penny, and my kids scripts are free....socialised medicine isn't that bad!

Mine is $10 per child per year. Even my baby who doesn't need camp or school forms or anythign filled out. Just the way of the times...

I pay $15 per form. Most of the info on the form is right off the chart so it's not like there is much "thought" to filling out the form. In fact, sometimes I think the nurse or assistant completes the form and the doctor just signs it, or even worse, stamps the doctors signature on it.
Oh, and if I want it mailed to me I have to address an envelope...the peds office springs for the postage though!

No charge if I have a form to be filled out at an appt.
When I need updated ones for daycare, I drop them off, they call when they are ready and DH usually picks it up and has to pay the $5.

I was charged $20 for the year at the old practice I took my kids to. You're going to see this more often.
The office where I work we can easily have 40 forms or more to complete by the end of the week. Some are very straight forward but some are very detailed and require more time to complete. Often times the provider stays after office hours to complete all these forms because some just can't be done in between patients. I think its a way to get reimbursement for the additional office hours spent in general on forms. Before I worked there I too was very upset at having to pay for forms but now I see how much time it takes because there are so many. Oh and in the summer its even worse because of all the school physicals, there are at least 20 forms/DAY that need to be completed. I do think charging PER form is a bit much but I think a small yearly fee is not much to ask for considering the extra time spent.

My ped office was GOING to start charging a year or 2 ago.. then they posted signs saying basically due to the economic crisis they are not going to start charging

Ours charges $10 per year per child. My pediatrician had to fill out a boatload of forms this year, so I really didn't mind paying it.
Well visits we do not pay for at all, but I think that has more to do with your health insurance than the doctor, though I could be wrong.
