Insulation
If the ceiling on the second floor of our house is insulated, would we need to insulate the attic above the second floor too?

No. We have our ceiling insulated/floor of the attic, but nothing on the underside of the actual roof. There's no need, since it's unheated space. That's how the majority of houses are insulated when the attic is unfinished.
