For my DD's birthday she got lots of clothes (size 24mos is a safe bet for a 1yo), a lot of toys (mostly Fisher Price toys geared to toddlers but a couple of stuffed animals as well) and cash.

GC to Borders or Barnes and Nobel
Clothing from Gymboree, Carters, Children's Place, or Baby Gap
Personally I like to give savings bonds and a book.

Gift cards, clothes, books are all good.
If you get toys, get something that they can always have more of--for a boy I would suggest a truck of some kind.

Both of my kids would have been swimming in 24m at 1 year old.
If you go the clothes route, ask the parent what size they're wearing. My two run pretty true to age. At 1, they were/are wearing 12m/12-18m. I had to exchange almost all of the clothing Erik received for his first birthday in May.
other suggestions:
a ride on toy
bubbles (always a big hit)
books
beach/pool toys

DD received a battery operated bubble blower for her 1st birthday - HUGE hit.
Also, books, she really likes the ones with flaps that you have to open.
