Direct number to the Fire Department if you need an ambulance
I saw you had that aldready.
I also include the pediatrician's service number. My Ped does not answer the phone in the office until 9:30am.

If your kids see any specialists - we include the allergist's number as well.
Pharmacy so that if a med needs to be called in it's easy to find.
ETA: for the one I leave on our fridge I have our address and home phone number. If it were an emergency a babysitter might not be able to remember it right away.

Pediatric dentist (when the time comes)
if this is for a babysitter, I have a form I used to use.
It included DC's current age, birthdate, weight & medical insurance info.

Barb, it's for our house but mostly for the days when DC's are sick, are home from daycare and MIL is watching them. I'll still put you on it... or maybe the real "twin expert"... your mom

This may sound elementary but you should have your full address, phone number and cross street on the list as well.
ETA: Nearest hospital # too.
