Ladies - can someone who's done the name change form attach the link on here for me please? I never changed from my maiden name and I want to have my certification be up to date. I found one form but it doesn't look right because it has lists of professions like Podiatrist, etc. - don't see anything on there about teachers, counselors, etc - or is it all the same form? All you have to do is sign up and create a username and then you can use the site. I changed my name on there. Posted 1/9/07 7:46 PM |

If you want new certificates with your new name...you must complete the application and mail it in along with a copy of your marriage cert. and $. It's on the highered website under forms and applications. Posted 1/10/07 9:18 PM |
